In this critical role, you will be responsible for delivering expert pharmaceutical care tailored to the unique needs of hospice patients. You will collaborate with coordinated care teams to ensure optimal medication management, enhancing the quality of life for patients and their families.

Required Qualifications: 

• Must be licensed as a pharmacist in the state of Texas 
• Preferred two (2) years of experience in a hospice pharmacy setting. 
• Bachelor of Pharmacy (PharmBS) or Doctor of Pharmacy (PharmD) degree.  

Daily Responsibilities: 

• Deliver expert pharmaceutical care to achieve optimal drug therapy results for patients. 
• Oversee the intake process and ensure the accurate and efficient fulfillment of prescription orders. 
• Apply and uphold drug utilization management practices and initiatives. 
• Guide and support staff in proper dispensing methods and medication verification processes. 
• Collaborate on technical and clinical departmental tasks to enhance accuracy and efficiency. 
• Provide training for staff on non-sterile compounding standards per state and federal regulations. 
• Ensure adherence to all pharmacy laws and regulations for compliance.
